RN Case Manager – Residential Setting
Job Summary:
The RN Case Manager in a residential setting is responsiblefor coordinating and overseeing the delivery of healthcare services toresidents. This role ensures that each resident receives individualized,high-quality, and cost-effective care. The RN Case Manager acts as a liaisonbetween residents, families, healthcare providers, and support staff to promoteoptimal health outcomes and continuity of care.
Key Responsibilities:
+ Assessment & Care Planning:
+ Conduct comprehensive health assessments upon admission and at regular intervals.
+ Develop, implement, and regularly update individualized care plans in collaboration with interdisciplinary teams.
+ Care Coordination:
+ Coordinate medical appointments, treatments, and follow-ups.
+ Communicate with physicians, therapists, and other healthcare providers to ensure cohesive care delivery.
+ Facilitate transitions of care, including hospital admissions and discharges.
+ Advocacy & Support:
+ Advocate for residents’ healthcare needs and preferences.
+ Educate residents and families on health conditions, medications, and self-care strategies.
+ Provide emotional support and counseling as needed.
+ Compliance & Documentation:
+ Maintain accurate and timely documentation in accordance with regulatory standards.
+ Ensure compliance with state and federal healthcare regulations and facility policies.
+ Team Collaboration:
+ Participate in interdisciplinary team meetings and case conferences.
+ Train and support residential staff on health-related procedures and protocols.
+ Discharge Planning:
+ Initiate discharge planning upon admission.
+ Coordinate with community resources and services to ensure a smooth transition post-discharge.
Qualifications:
+ Current RN license in the state of practice.
+ Minimum of 2 years of nursing experience, preferably in case management or residential care.
+ Strong knowledge of chronic disease management, mental health, and community resources.
+ Excellent communication, organizational, and problem-solving skills.
+ Ability to work independently and collaboratively in a team environment.
