Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice is looking to add a dedicated and compassionate Admissions Nurse to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This role will oversee the Mercer County areas.

The position will work the following schedule:

Friday - 3:00 PM - 11:00 PM

Saturday - 10:00 AM - 10:00 PM

Responsibilities:

+ Conduct timely, comprehensive assessments to determine hospice eligibility based on clinical criteria and Medicare guidelines.

+ Clearly and compassionately explain hospice philosophy, services, and benefits to patients and families.

+ Initiate the admission process and obtain all necessary consents, documentation, and physician orders.

+ Coordinate with the hospice interdisciplinary team to ensure seamless onboarding and transition of care.

+ Provide patient and family education on symptom management, medications, and what to expect during hospice care.

+ Collaborate with hospitals, nursing homes, physicians, and referral sources to facilitate smooth transitions into hospice.

+ Accurately document all assessments, findings, and communications in the electronic medical record (EMR) in a timely manner.

+ Maintain compliance with federal, state, and organizational policies and procedures.

Requirements:

+ Current NJ RN License in good standing

+ Hospice Experience Preferred

+ Willing to travel to the assigned areas

+ Must have reliable transportation with liability insurance

+ Strong communication and interpersonal skills.
