The Assisted Living Program of The New Jewish Home is seeking a full time ( 37.5 hours per week) Case Manager to join our team!
JOB SUMMARY: The Case Manager of the Assisted Living Program (ALP) is a key member of the ALP interdisciplinary leadership team, focused on individualized resident services that ensure the specific needs and preferences of each are met. Day to day responsibility include the operation of a case management system providing for initial and ongoing assessment of resident needs and coordination of care.
The Case Manager reports to the Director of the Assisted Living Program.
DUTIES AND RESPONSIBILITIES:
+ Participates in pre-admission activity, including meeting with prospective residents and their representatives to discuss matters described in the residency agreement and regulatory required disclosures, including the services that the residence can provide to meet the needs of the prospective resident
2. Identifies and evaluates the resident’s needs, interests and strengths and the capacity of the facility to meet those needs prior to admission.
3. Assists with new resident adjustment to the facility including orientation of the new resident and representative(s) to the general daily routine.
4. Assists each resident to maintain family and community ties and to develop new ones. Assist each resident to strengthen the informal relationships among residents. Encourages resident participation in facility ad community activities.
5. Establishes linkages with and arranges for services from public and private sources for income, mental health and social services. Specifically, ensures prompt and accurate completion of resident applications and (re)certifications for Medicaid, SSI and other financial assistance programs.
6. Provides ongoing case management services to each resident for the duration of his/her residency by identifying and evaluating the resident’s needs, interests and strengths and the capability of the facility to meet those needs at lease once every 12 months, using a specific evaluation form.
7. Documents appropriately and in a timely manner in resident record, which may include, but is not limited to, specific 1:1 visits, group work, mental health updates and psychosocial interactions.
8. Completes appropriately and in a timely manner the case manager portion of the resident’s specific plan of care in accordance with regulatory guidelines.
9. Coordinates with the Nursing Department to ensure an initial mental health evaluation (MHE) is completed prior to admission for all residents with a mental health diagnosis or a psychotropic medication prescription.

REQUIREMENTS
+ A master’s degree in social work from an accredited college or university, or a bachelor’s degree from an accredited college or university with major work in human services or service delivery and one year of full time experience in the provision of services to a frail adult population
SPECIALIZED SKILLS AND COMPETENCIES
1. Computer skills including ability to perform basic functions in word processing, spreadsheets, email and electronic medical record use
2. Ability to read, write and speak effectively to staff, residents, visitors and community representatives
3. Ability to follow directions, apply common sense to problem solving and have excellent communication skills
4. Ability to speak/write Spanish.
AGE RELATED SKILLS AND COMPETENCIES
1. Ability to communicate and interact well with the adult and elderly population.
JOB QUALIFICATIONS
1. A master’s degree in social work from an accredited college or university, or
2. A bachelor’s degree from an accredited college or university with major work in human services or service delivery and one year of full time experience in the provision of services to a frail adult population; or
3. An associate’s degree from an accredited college of university with major work in human resources or service delivery and three years fulltime experience in the provision of services to a frail adult population.
PHYSICAL DEMANDS
1. Standing on feet and/or sitting for long periods of time.
2. Pushing wheelchairs, moving objects such as tables, chairs and equipment.
