LPN Case Manager -** **Marjorie Doyle Rockwell Center in Cohoes, NY
We have an opportunity for a LPN to further their nursing skills as a leader and develop strong bonds with their residents and coworkers! The team is looking for a **LPN case manager to work full time** and who is motivated to help bring leadership to a diligent and passionate team and provide top quality care!

What you will do:
Eddy Memory Care at Marjorie Doyle Rockwell Center is a memory care community **complete with 52 private and semi-private rooms** providing comfortable, secure living in an environment designed for safety and independence. Programs serve individuals with early to mid-stage memory impairment — those with difficulties that require watchfulness and a supportive environment, but **who do not need nursing home care** . The center is thoughtfully designed and **services are tailored for individuals with memory loss, Alzheimer’s disease and other related dementias** .

Responsibilities:
+ The LPN Case Manager assists the Assistant Administrator with the overall operation of the facility
+ The LPN Case Manager will help coordinate care for residents who are functionally, physically, and/or socially impaired and need 24 hour oversight
+ Provides leadership and acts as a resource to clinical staff
+ Understand and be empathetic to the needs of the geriatric population and be polite and responsive to residents at all times
+ Effectively plan and organize assignments in the most efficient manner
+ Work flexible hours, provide oncall coverage, and participate in holiday activities on occasion
What you will need:
+ Valid NYS LPN license
+ 2+ years of experience as a LPN
+ Experience in a leadership or charge role
+ Experience working in memory care as a LPN highly desired
+ Experience with scheduling; Kronos Time Management system
+ Maintain the highest level of customer service while completing day to day duties and responsibilities
+ Communicate effectively in a fast pace work environment
+ Can remain calm and think clearly in emergency situations, responding and acting in an appropriate manner
+ Engage within a team setting
+ Strong leadership and mentorship of the Resident Assistant and LPN staff
+ Passion to learn, grow and develop in the nursing field
Pay range: $25.00-$31.45
_Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location_
