    Description:

    POSITION PURPOSE

    + The Clinical Practice Leader (CPL) acts as a member of the nursing leadership team responsible for driving safety and ensuring quality for patients and colleagues at the unit level.

    + The CPL serves as a change agent to translate and integrate evidence into practice; to design and evaluate care and improve outcomes.

    + As the process owner for nursing practice and quality, the CPL focuses on key nurse-sensitive indicators for improvement, utilizing process improvement methods to drive best practice.

    UNITS SUPPORTING

    2 East Progressive Care Medical Surgical Unit

    + 2 East is a 32-bed unit that provides care to cardiac, thoracic, vascular, other surgical patients and overflow of medicine patients.

    + Care of the cardiothoracic patients through the continuum, cardiac monitoring, arterial lines, vasoactive/antiarrhythmic drips, various lines/tubes and drains, and prompt response to patient emergencies

    + Patients on this unit require close and frequent assessments as well as skillful and timely interventions in order to achieve their treatment goals.

    + Care is provided by a team of physicians, advanced practice nurses, physician assistants, registered nurses, respiratory therapists, pharmacists, social workers, case managers, dietitians, and therapists, ensuring holistic and specialized care.

    WHAT YOU WILL DO

    + Performs and oversees the collection of holistic assessments and the development and implementation of a plan of care. Assesses patient’s response to treatment and care in collaboration with other care providers.

    + Consults on complex problems in client care, documents findings and progress in electronic record. Consults with physicians and other healthcare providers to communicate patient progress.

    + Ensures patient/family preparation for care across continuum prioritizing for high risk, chronically ill, complex, medically fragile patients.

    + Demonstrates critical thinking skills by synthesizing multiple data elements to evaluate and achieve optimal patient outcomes.

    + Assimilates and applies research/evidence-based practice information that supports improved patient care outcomes.

    + Collaborates as a member of unit leadership to achieve effective nursing operations:

    + Leads and or actively participates in daily quality huddles

    + Formally coaches and reinforces identified behaviors and established expectations related to nursing practice

    + Leads, supports and participates in shared governance activities

    + Identifies and substantiates resources needed for optimal patient care. Looks for opportunities to reduce costs and waste

    + Leads clinical practice at the unit level to achieve unit quality improvement and outcome-based initiatives. Initiates unit-based system reviews such as learning from defects and participate in root cause analyses to improve the quality of patient care delivery; critically evaluates and anticipates risks to patient safety.

    + Supports the clinical and professional development of nursing colleagues through mentoring, coaching and role modeling, including residency program and project development.

    MINIUMUM** **QUALIFICATIONS

    + Education: A Master's of Science in Nursing (Clinical, Education, Administration all acceptable) or a Bachelor of Nursing degree with enrollment in a nursing program to obtain a doctorate in nursing (DNP) with completion of DNP degree within 6 months of hire.

    + Experience: Three to five years of acute care nursing for similar patient population. Progressive experience in health care including involvement in direct patient care, educating staff, supervising staff, and problem solving of unit or organizational issues.

    LICENSURE / CERTIFICATION:

    + Current licensure as Registered Nurse in state of MI

    + Appropriate certification in area of specialty within 1 year of hire—as related to graduate degree
