What You’ll Do:
+ Deliver skilled nursing care based on physician orders and supervised RN direction
+ Administer medications, treatments, and procedures with precision and empathy
+ Monitor client health and report changes to ensure timely, appropriate interventions
+ Educate clients and families to empower them in their care
+ Collaborate with an interdisciplinary care team to create and adapt care plans
+ Maintain accurate, timely documentation that reflects your expert care
+ Stay up to date on best practices, standards, and regulations—because excellence matters

What You’ll Need:
+ Graduate of an accredited LPN program
+ Current, active state licensure
+ 1+ year of experience in an acute care setting (home care preferred)
+ Certification to administer medications (NAPNES or equivalent)
+ Reliable transportation and a valid driver’s license
+ A heart for service and strong communication skills
Physical Requirements:
+ Frequent walking, lifting (up to 35 lbs), and assisting with patient mobility
+ Ability to manage shifting priorities and a fast-paced care environment
+ Flexibility to travel between client homes and work sites
