The Referral/Enrollment Specialist is responsible for engaging with the patient and referral source. Reviews incoming referrals for appropriateness for palliative care or hospice. Processes and posts information to patient’s chart, educates patients and families regarding medications, treatments and care, and referrals. Manages patient phone calls, triage, and follow-up under directions of providers and clinical staff. Relays appropriate information and interacts with referral sources to facilitate program services. Provides a high level of administrative and provider scheduling support and general clerical and project-based work for both the hospice and palliative care groups.
