    Summary Works for the Central Virginia VA Health Care System in Richmond, Virginia, as a direct report to the Social Work Service, and is assigned to the Medical Foster Home (MFH) Program within Geriatrics & Extended Care (G&EC). Responsibilities This position is responsible for overall MFH Program development, management, operations, policy development and evaluation; ensuring compliance with all applicable program requirements. It is the MFH Coordinator's responsibility to review regularly VHA Directive 1141.02(1), Medical Foster Home Program Procedures, to ensure all tasks and requirements in the directive are being met. Develops and implements information systems to track service activities for the MFH Program including visits, referrals, days of care, Veteran demographics, and Veteran satisfaction as needed. This includes: developing and/or maintaining current program documents (i.e. MFH Caregivers Guide); maintaining records of VA inspections including maintaining a file of records for each MFH; and submission of reports and data for CVHCS, VISN, and national use (i.e MFH Monthly Report). Will provide public outreach, which includes marketing MFHs to VA staff, Veterans and families, Veterans Service Organizations, and the community; establishing working relationships with internal and external stakeholders (i.e. meeting quarterly with the Home Based Primary Care (HBPC) and/or SCI Home Care Director or designee(s)); as well as recruiting of MFHs and MFH caregivers. The MFH Coordinator is also responsible for developing and planning of MFH Sponsor educational programming. Responsible for the screening of potential new MFH facilities, coordinating of VA inspections of MFHs, ensuring deficiencies are corrected within appropriate timeframes and that MFHs maintain adherence to program criteria, and conducting unannounced visits to each MFH monthly. Administers program implementation to enhance the quality of service provided to Veterans. Activities include overseeing Veteran referrals to the program and facilitating their transitions into a home and actively monitoring and reporting on quality of care and adverse events. Responsible for ensuring the reporting and investigating of sentinel/adverse events by the program staff as necessary. Maintains level II certification as a Contract Officer Representative (COR). If the incumbent is not currently certified at a level II COR they will work to achieve this certification within 2 years. Qualifications To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 06/02/2025. Social Worker, GS-12 Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure/Certification. Individuals assigned as Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. In addition to the experience above, the candidate must demonstrate all of the following: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120, Part II, Appendix G39. The full performance level of this vacancy is GS 12.
