Preferred Qualifications
Role Purpose
The Value-Based Care Consultant (RN) works directly with value-based care (VBC) performance leadership on all set organizational quality and utilization goals as it relates to value-based contracts. This position is responsible for engaging providers and offices in a data driven approach to implement and optimize value-based care workflows to improve population health management efforts, including cost, quality, access, network optimization, and documentation and coding.
Responsibilities
+ Serve as a subject matter expert on population health and value-based care in assigned health plan programs and initiatives, acting as a resource for others within the organization
+ Provide clinical and care coordination support to practices and patients within value-based care contracts towards improving overall quality of care and contract performance
+ Support the successful implementation of population health management initiatives and/or programs at the practice level and within centralized structures
+ Review health plan reports; monitor performance on program metrics, quality outcomes and adherence to contract requirements
+ Identify performance improvement opportunities through analysis of operational data, clinical outcome data and utilization/claims data
