Hours:Monday - Friday
Rate of Pay:$45 per hour
OVERVIEW OF PRIMARY RESPONSIBILITES :
CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Under the supervision of the CTI Team Lead, in addition to Care Management tasks, the CTI Nurse will assist in resolving medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The Nurse will work on a Full-Time basis and using a hybrid work model, working in the community between several locations and counties including but not limited to Sullivan, Orange & Rockland Counties, NY.
PRIMARY FUNCTIONS :
+ Carry a primary caseload providing CTI services.
+ On-call responsibilities one week per month.
+ Assess basic health and medical needs within their scope of practice.
+ Coordinate health care with community medical providers.
+ Provide rehabilitation, education, and support services.
+ Works to create relationships with key providers in the community.
+ Regular meetings with CTI LPN
+ Other duties as assigned by supervisor.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
+ Participate in rotating on-call responsibilities one week per month.
+ Complete documentation and reporting in alignment with agency standards.
+ Work both independently in the field and collaboratively in a hybrid team office setting.
+ Use agency vehicle or personal vehicle (mileage reimbursed) for client transportation and field visits.
QUALIFICATIONS AND ATTRIBUTES :
+ Strong organizational and time management skills.
+ Effective problem-solving and critical thinking abilities.
+ Capacity to manage and prioritize complex caseloads.
+ Demonstrated ability to work across systems (healthcare, housing, disability services).
+ Empathetic, client-centered approach with strong interpersonal communication skills.
+ Spanish language proficiency is a plus.
EDUCATION AND EXPERIENCE :
+ Must be a Licensed Registered Nurse currently registered by NYSED.
