    Overview

    Endocrinology – Diabetes Care APP

    Dartmouth Health Cheshire Medical Center is seeking an NP or PA with a strong interest in diabetes care to join our endocrinology team in Keene, NH. Our focus is on empowering patients to manage their condition through personalized treatment plans, advanced technology, and patient-centered care.

     

    The successful candidate will provide expert clinical management, education, and support for patients with Type 1, Type 2, and gestational diabetes. You will work closely with endocrinologists, primary care providers, and a multidisciplinary team to deliver comprehensive, individualized care.

    Key Responsibilities

    * Patient Care: Conduct comprehensive assessments, including medical history, physical exams, and diagnostic evaluations for diabetes management.

    * Treatment Plans: Develop and implement personalized diabetes care plans, including medication adjustments, insulin therapy, and lifestyle interventions.

    * Patient Education: Provide detailed education on self-management, blood glucose monitoring, medication use, nutrition, and exercise.

    * Technology Integration: Support patients in using continuous glucose monitors (CGMs), insulin pumps, and other diabetes management technologies.

    * Care Coordination: Collaborate with primary care providers, endocrinologists, dietitians, and behavioral health specialists to deliver integrated care.

    * Monitoring and Follow-Up: Regularly evaluate treatment effectiveness, adjust plans as needed, and ensure continuity of care.

    * Preventative Care: Identify and manage risk factors for diabetes complications, including cardiovascular health, eye care, and foot health.

    * Documentation: Maintain accurate, timely, and comprehensive patient records in compliance with regulatory standards.

    Qualifications

    * Graduate of an accredited APRN or PA program

    * Current and unrestricted license as a Nurse Practitioner or Physician Assistant in New Hampshire

    * NCCPA certification or board-certified by the AANP or ANCC

    * Passion for endocrinology and diabetes care

    * Compassionate and patient-centered approach to care

    * Clinical experience in diabetes care preferred
