Overview
This position is eligible for a Sign On/ Retention Bonus fo $5,000.
Evaluates, assesses, and plans nutrition care to meet patient nutritional needs.
Responsibilities
* Assesses patient nutritional status using documented medical information and established nutritional standards.
* Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
* Identifies inpatients at nutritional risk and documents recommendations.
* Diagnose and develop intervention for patients with malnutrition
* Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
* Conducts education programs for patients, community groups, and staff.
* Precept and mentor new staff or existing staff in clinical specialty area.
* Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
* Cross-covers a broader spectrum of patient care areas and subspecialties as needed.
* Performs other duties as required or assigned.
Qualifications
* Bachelor’s Degree from an accredited program in nutritional sciences (prior to 2024).
* Master’s Degree from an accredited program in nutritional sciences (2024 and after).
* Minimum 2 years’ related experience preferred.
Required Licensure/Certifications
- Current NH Dietitian license.
- Current Registration by the Commission on Dietetic Registration.
