Position Summary
The Population Health Manager collaborates with a multidisciplinary staff to meet Population Health and quality improvement goals through projects aligned with the mission of Apicha and its strategic goals. The Population Health Manager supports the implementation of strategies to drive clinical transformation toward value-based care delivery and addressing underlying health disparities and social determinants of health. The role encompasses analysis and review of trends and projections of key clinical metrics, supporting new program development, and making recommendations to ensure the viability of the organization over the short and long term. The position requires an understanding and awareness of a dynamic healthcare landscape, including evidence-based practice, value-based payment, and delivery system transformation in order to achieve high quality of care.
The major duties of the position include, but are not limited to:
Project Management
+ Assist in the development of project scopes and statements of work under the guidance of the Director of Population Health Management
+ Monitor project progress by tracking milestones, resolving problems, and coordinating status update meetings with stakeholders
+ Development of workplans and protocols designed to guide team activities, ensuring delivery of key performance initiatives and goals
+ Coordinate data collection and reporting of member enrollment and quality performance indicators with Managed Care Organization value-based initiatives and related reviews
Data Analysis and Validation
+ Analyze complex healthcare data from multiple, disparate sources to identify best practices, care delivery trends and clinical practice variation
+ Utilize Excel and/or business intelligence tools (Tableau, Power BI, SAS) in the development of presentations, research abstracts, and agency-wide reports
+ Validate data using various electronic health record and population health management systems and identify data discrepancies with demographic or clinical information
+ Support efforts to ensure data integrity and validity, and perform ongoing quality assurance of data and reports, including NCQA PCMH accreditation, HEDIS/QARR, UDS, and HIV Treatment Cascade
+ Validate data accuracy of Population Health dashboards supporting improvement of patient health outcomes
+ Support implementation process and data optimization efforts to ensure data is accurate and reliable for use in performance improvement efforts
Quality Improvement
+ QI/QA activities shall be a part of the PHM's daily responsibilities.
+ Ensure implementation and update of QI/QA operating procedures and completion of assessments and reporting QI/QA outcomes
+ Lead QI initiatives by applying quality improvement methodologies (i.e., RCA, PDSA, DMAIC) and assists clinical and non-clinical teams in developing appropriate tracking metrics to monitor performance and ensure compliance with the Apicha QI/QA Plan
+ Assist Director of Medicine in the performance of root cause analyses and recommends appropriate actions to improve clinical and non-clinical outcomes; develops visual presentations of current QI projects and accomplishments
+ Collaborate with clinicians and staff to conduct quality assurance activities to meet various external reporting requirements including PCMH accreditation, HIVQUAL, UDS and RSR
Staff Management
+ Supervises Quality Improvement Coordinator (QIC) in meeting project goals and performance outcomes
Qualifications:
+ Bachelor’s degree in Nursing, Health Care Administration, or related field required; advanced degree in related field, such as MS, MPH, or MA, strongly preferred.
+ Experience managing projects in health care quality improvement, performance improvement, and/or practice transformation preferred.
+ Intermediate to advanced analytic skills and knowledge of research methods, data collection, and analytical methodologies
+ Ability to adapt with evolving Population Health quality performance standards and measures
+ Familiarity with medical terminology, ICD-10, and CPT coding strongly preferred
+ Understanding of UDS, HEDIS, and NCQA PCMH accreditation quality standards
+ Excellent verbal and written communication skills; detail oriented; excellent proofreading and editing skills
+ Strong working knowledge of Microsoft Office applications (Word, Excel, PowerPoint)
+ Ability to work independently and in a team environment; ability to work with multidisciplinary staff with diverse backgrounds
+ Ability to stand, walk or sit for an extended period of time
+ Ability to hear within normal range
+ Ability to see within normal range
+ Noise level is moderate
