RN Nurse Navigator - Bon Secours Liver Institute of Richmond - Richmond, VA
This is a full time hybrid position.
Summary of Primary Function:
Coordinates the care of patients during all phases of care from referral or diagnosis through post-care. Provides high quality, comprehensive and cost-effective nursing care for both patients and their families consistent with established standards. Facilitates the transition of patients from inpatient to outpatient and vice versa throughout all phases of care. Serves as care coordinator for the multi-disciplinary team. Primary contact for access to services provided by care team. The RN Nurse Navigator is a self-directed individual whose primary function is that of nurse clinician, with essential education, consultant, case management, and leadership role components.
Essential Job Functions:
+ Provide liver transplant coordination, establish and maintain good working relationships with other facilities and departments to help facilitate the pre-liver transplant evaluation process Monitor lab results and communicate with providers, patients, and transplant centers regarding results Assemble transplant pre-evaluation documentation and prepare liver transplant evaluation summary Coordinate with transplant centers Coordinate care of patients post liver transplant to ensure compliance with visits, labs and procedures; assist providers in management of patients with rejection, HCV recurrence and other complications post-transplant Review patient labs, assist with medication management, respond to patient calls Maintain close contact with Liver Institute providers, patients and transplant coordinators at other centers
+ Liver Cancer (HCC) care coordination for embolization treatments along with radiology coordinate immunotherapy track labs coordinate with other departments (ie, Specialty Pharmacy, Infusion, Lab, Interventional Radiology) Discuss adverse events of medication with patient by phone Schedule follow-up scans to monitor liver cancer response to treatment. Work with liver transplant programs regarding HCC monitoring and upgrade status for LT because of HCC.
+ Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
+ Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
+ Provides patient care to adolescents, adults, geriatrics
+ Work closely with specialty pharmacy.
+ Provide clinical information for denied procedures ordered and arrange for peer to peer as needed.
+ Complete paperwork for patients (i.e. FMLA, disability) in conjunction with provider
+ Assist with emergency coverage as needed at other Liver Institute locations.
+ Maintain licensure and provide licensure and primary source verification when renewed prior to expiration date.
+ Keep BLS current and provide card to practice administrator upon completion.
+ Complete all annual training prior to deadline.
+ Performs all assigned duties in a manner consistent with appropriate customer service requirements and in support of Bon Secours Mercy Health mission, Code of Conduct, and values.
+ Assist with hepatocellular and liver transplant clinical trials or observational studies as needed.
+ Attend provider and staff meetings as well as educational conferences within Liver Institute
+ Regulatory compliance (ie JCAHO, OSHA, Provider-based)
+ Other duties as assigned.
+ Adheres to St. Francis and Bon Secours rules, regulations, and policies.
+ Demonstrates expertise in critical thinking and nursing problem solving skills based on theoretical knowledge, clinical expertise, and sound judgment.
+ Serves as a clinical resource for nursing practice in area of specialty.
+ Performs duties and responsibilities of a nurse clinician as required.
+ Assesses health status by interviewing patient regarding current status and medical history.
+ Collaborates with social work/case management to identify psychosocial, educational and discharge needs of patient/caregiver.
+ Works as a multi-disciplinary care team member to ensure access to hospital and community resources as needed.
+ Provides psychosocial support to patients and families throughout pregnancy and post-partum.
+ Facilitates communication among patient/family, physician and interdisciplinary team through utilization of regular case conferences and interdisciplinary rounds.
+ Participates in the development and implementation of standards for obstetrical patients in conjunction with other health care professionals.
+ Communicates with the financial coordinator/Medicaid enrollment counselor to determine Medicaid eligibility and assist the uninsured patient to obtain health insurance coverage as well as identify sources of financial aid if needed.
+ Assists the unassigned patient with access to the OB physician office for follow up from the OB emergency department for establishment of care.
+ Incorporates standards of care associated with the obstetrical program into clinical practice.
+ Assists with development, implementation, communication, and education of obstetrical navigation program to other health care professionals and their associated medical practices, community resource agencies, and patients/family.
+ Participates in the development, implementation, evaluation, and revision of policies and procedures/ protocols related to the obstetrical navigation program.
+ Assists with maintaining regulatory compliance, quality assurance, and data management as needed including, but not limited to, EMTALA, MSE, Birth Certificate, DHEC Newborn Screenings.
+ Patient and family navigation through the perinatal palliative care process will occur for pregnant patients expecting newborns with known lethal anomalies.
REQUIRED QUALIFICATIONS:
+ 2 years/associate degree, Specialty/Major: Nursing
+ At least 2 years nursing experience in community nursing, critical care or emergency department experience preferred.
+ Proficiency in basic computer skills, including the ability tomaneuver within a Microsoft Windows environment.
PREFERRED QUALIFICATIONS:
+ 4 years/bachelor’s degree, Specialty/Major: Nursing
+ Licensure Required: Current Commonwealth of Virginia nursing license
