    Job Brief

     

    Ensures the overall care and well being of persons receiving services in assigned residences.

    Shift Worked:

    9:00AM - 5:00PM (Flexible)

    Weekly Hours:

    37.5

    $27.17 - $30.76

    POSITION SUMMARY:

    Ensures the overall care and well being of persons receiving services in assigned residences. Responsible for the overall management of the residences and direct supervision of Shift Supervisors or Assistant Residential Coordinator and Direct Care Counselors, Sets performance standards and provides the support and training necessary for staff to achieve standards.

     

    B. POSITION DUTIES:

    1. Interviews, hires, evaluates staff’s performance, supervises, provides feedback and disciplines Direct Care Counselors, and Shift Supervisors in assigned residences. Documents all supervisory face-to-face meetings.

    2. Provides initial orientation to all new staff and coordinates ongoing training and in-servicing as needed.

    3. Reviews staff schedules with Staffing Coordinators and ensures that all direct care schedules are accurate and efficient with a sign-off to meet the needs of people who receive services. Ensures adequate staffing is available for all necessary clinic appointments. Utilizes Time Care system to complete payroll.

    4. Schedules and conducts routine quarterly cluster meetings and as needed with all shifts of Direct Care Counselors, Nurse, and Clinical Team.

    5. Responsible for scheduling all annual medication re-certifications with the Nurse Case Managers or designated RN’s and ensuring documentation is maintained on site.

    6. Ensures all employee accident reports are completed and submitted to Human Resources in a timely manner. Completes all supervisory investigations.

    7. Ensures compliance to fiscal year budget in personal services and other expenses for assigned residences.

    8. Responsible for all financial systems for the people who receive services, including personal funds, household monies, and recreation funds. Completes monthly review and sign-off of each of the above notes and ledgers. Ensures personal funds for the people who receive services.

    9. Responsible for the completion of all incident reports and all necessary follow-up. Attends Incident Review meetings when needed. Ensures incident review minutes are on site.

    10. Ensures that fire drills for each shift are implemented and reviewed prior to forwarding to the Administrator for signature and ensures all documentation is on site (i.e. sprinkler reports, quarterly inspections, etc.)

    11. Responsible for overall maintenance and cleanliness of the assigned residences, completes a biweekly and as needed walkthrough pending site concerns. Ensures that all necessary purchase orders and work orders are generated and ensures timely follow-up of each. Reviews housekeeping cleaning form and conveys any problems to the Director of Residential Services.

    12. Ensures that the Service Plan/Outcomes for people who receive services is implemented and participates in conferences and special team meetings according to regulatory compliance.

    13. Performs all other duties as assigned.

    C. REQUIREMENTS:

    1. Bachelor's degree in a related Human Services field is preferred or a minimum of five (5) years of experience working in the Intellectual and Developmental Disabilities (ID/DD) field.

    2. Minimum of two (2) years supervisory experience required.

    3. Must have knowledge of HCBS Waiver, IRA and ICF regulations, Quality Management.

    4. Must be able to provide weekend coverage as scheduled and work flexible hours as needed.

    5. Working knowledge of Microsoft Office Applications and data entry.

    6. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all Corporate Compliance/Quality Management investigations and reviews.
