    We are seeking a talentedProgram Managerto join our team of nearly 500 employees. Under the guidance of CCSI and partners from the Monroe County Office of Mental Health, theProgram Managerwill oversee subcontracted vendors who are delivering direct services to individuals enrolled in the program, ensure that all grant requirements and deliverables are fulfilled and provide ongoing outreach and consultation to community partners to encourage referrals and raise awareness of program services and eligibility criteria. As part of their duties, theProgram Managermaintains connections with other community partners and referral sources to promote the availability of C2S program and assist with engaging individuals who may benefit from these services.

     

    This position is funded by a 5-year grant from the Substance Abuse and Mental Health Services Administration (SAMHSA). CCSI is the primary recipient of this grant, which is currently in Year 2, in partnership with the Monroe County Office of Mental Health.

     

    CCSI is partnering with Monroe County Office of Mental Health to hire aProgram Manager. This position is employed by CCSI and works on behalf of Monroe County Office of Mental Health.

     

    In this role, you will

     

    + Manage grant program operations by conducting meetings, promoting open communication among stakeholders, and resolving ongoing problems and concerns of consumers, community partners and staff.

    + Facilitate collaborative processes and maintain communication among service providers and with the community.

    + Oversee grant budgeting and finances, as well as grant and contract renewals and future consideration of sustainability efforts.

    + Monitor grant budget and subcontractor invoicing to ensure spending stays within available resources.

    + Develop and reinforce project operational policies, procedures, protocols, workflows, MOU’s, contracts, and partner relationships.

    + Support evaluation of grant program by collaborating with evaluation staff and developing/implementing efficient and effective data collection strategies.

    + Participate in analysis and reporting of results and recommend and/or take appropriate action(s).

    + Receive referrals, confirm eligibility and triage referrals to provider partners in a timely manner. Participate in team case review meeting and assist with problem solving.

    + Serve as C2S program point of contact for community touch points such as shelters, emergency rooms and other housing programs in Monroe County.

    + Provide information about the C2S program to other providers and community support programs to increase awareness of the program and eligibility criteria, including in-person and virtual presentations as well as in person and phone-based consultation.

    + Facilitate referrals to the C2S program, including working with individuals, families, and providers to determine eligibility and ensure that the necessary information is provided to initiate services.

    + Be responsible for timely submission of all data and reporting as required by grant source.

    + Fulfill other responsibilities as assigned.

    Requirements

    What You Bring

     

    + Bachelors in mental health, social work, or related human services and 5 years’ experience in a related field, or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) )

    + Knowledge of homeless services system preferred.

    + Experience with Windows based operating system (Word processing, spreadsheet and database management software)

    Required for this position are:

    + Satisfactory completion of all required background screenings.

    + Local travel as necessary to maintain regular contact with community partners and referral sites.

    + Valid Class D New York State Driver License and access to motor vehicle to meet transportation requirements of position
