    LaSante Health Center is seeking Telehealth NP/PA to deliver essential healthcare services to patients through telemedicine platforms. This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.

    Responsibilities :

    + Plans and organizes patient care according to protocols and pathways.

    + Collaborates with the healthcare team to develop and implement treatment plans.

    + Conducts health assessments and orders necessary tests.

    + Prescribes medications and medical equipment as per practice protocols.

    + Evaluates treatment effectiveness and modifies plans as needed.

    + Promotes professional growth and serves as a clinical resource.

    + Assists Clinical Access Center staff when necessary.

    + Completes additional tasks as directed by supervisor

    Qualifications:

    + License: Current registration as Nurse Practitioner in New York State

    + Education: Master's degree in nursing

    + Certification as an Adult or Family Nurse Practitioner
