    Position Summary

    This has been designated as an essential position responsible for helping individuals with disabilities in navigating the systems and provision of available services, supports, and resources to meet individualized needs, as well as identifying and addressing any barriers preventing individuals from accomplishing their goals. The Hope Navigator is also responsible for connecting individuals to supports and services, in a timely manner, to address immediate or critical needs to stabilize the individual seeking assistance.

     

    The Work You Will Do

     

    + Conducting assessments to identify short-, medium-, and long-term goals and barriers to achieve those goals. Formulating a care plan that clearly illustrates actionable steps and timeframes developed in partnership with the individual in need and caregiver, if appropriate, to address areas of need and connect the individual with resources to achieve their goals. The Hope Navigator shall provide the individual with referrals to local, community-based partners that can assist in providing services that address barriers and identify supports to help the individual achieve their pathway to possibilities.

     

    ​​​​​​​

     

    + When appropriate, providing connections to immediate supports in a timely manner to stabilize individuals and families and mitigate crisis situations.

     

    ​​​​​​​

     

    + Assisting individuals in accessing available programs, services, and optional methods for reaching goals. This includes providing warm referrals to other state agencies, local community-based providers or services, and resources to support individuals and their families.

     

    ​​​​​​​

     

    + Providing follow-up and monitoring of connections to service delivery and assessing outcomes and results. Building and maintaining relationships with respect, trust, sensitivity, and confidentiality to the individual, coworkers, community, stakeholders, and partners.

     

    ​​​​​​​

     

    + Identifying and maintaining a working knowledge of available community resources to meet the needs of individuals with disabilities and connecting individuals and families to relevant community resources and support.

     

    ​​​​​​​

     

    + Actively participating in trainings and team meetings on an ongoing basis including required meetings regarding specific cases and attending other team or sister agency teaming meetings.

     

    ​​​​​​​

     

    + Understanding the resources available from state government programs and connecting individuals to those services they need and qualify for.

     

    ​​​​​​​

     

    + Documenting all pertinent information and ensuring that forms are completed and uploaded into agency business systems accurately and timely. This includes preparing correspondence, case notes, narratives, technical reports, and other documentation.

     

    ​​​​​​​

     

    + Perform other duties as assigned.

    Minimum Qualifications

    + Must have knowledge and be proficient in-service orientation.

     

    ​​​​​​​

     

    + Must have at least two years of experience in working with people with disabilities, care coordination, or in the health and human services field.

     

    ​​​​​​​

     

    + Must have a valid Driver’s License or other efficient means of transportation to travel for work purposes.

     

    ​​​​​​​

     

    + Must have a high school diploma or its equivalent.

     

    ​​​​​​​

     

    + Must be able to work during emergency situations or natural disasters and may be required to work before, during and beyond normal work hours/days.

     

    ​​​​​​​

     

    + Must have knowledge and proficient use of Microsoft applications (Excel, Word, Power Point Share Point) and other required computer software applications.

     

    This position is essential and requires overnight travel, flexibility in the work schedule (including non-traditional business hours) to work during emergency situations or other natural disasters.

    Must be:

    + Willing and able to attend disaster recovery sites that include distribution of supplies, canvassing, customer assistance?

    + Willing and able to travel throughout the state for essential work responsibilities?

    + Willing and able to travel on short notice?

    + Willing and able to work beyond normal work hours and days, as needed?

    Direct Deposit Program

     

    As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

     

    Background Screening Requirement

     

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

     

    No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

     

    Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.

     

    Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.
