We are currently seeking a passionate individual for the position of Full-Time Mental Health Housing Specialist. In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program.
POSITION RESPONSIBILITIES
+ With Administrator, assists residents with the development and implementation of a long-range service plans
+ Assists in securing and maintaining entitlements: SSI/SSD, PA Medicaid, SNAP, Section 8 housing, etc.
+ Conducts weekly home visits and one phone contact with assigned residents and additional support to resident(s) in crisis as needed and provides appropriate documentation of all contact
+ Responsible for contact and documentation of such with collateral service providers at least once a month and as needed
+ Assesses and provides referrals to residents regarding any unmet service needs
+ Attention to providing and modeling the necessary development of ADL skills: budgeting, cooking, household maintenance, personal hygiene, use of public transportation, etc.
+ Assists in the transportation of residents to MH programs and events
+ Responsible for assuring good relationships with neighbors and the general community
+ Ensures that room/apartments are clean for new residents (may include cleaning, moving and setting up)
+ Responsible for reporting all housing maintenance issues through the appropriate procedure
+ Responsible for the accurate and current maintenance of phone log, in addition to resident records
+ Immediately notifies Supervisor of all potential crisis/emergency situations
+ Documents all incidents and occurrences
+ Conducts resident house meetings, as needed
+ Conducts fire drills at each residence bi-monthly
+ Responsible for assisting residents at the time of moving in/out
+ Meets weekly for supervision
+ Reviews residents rent monthly with Supervisor
+ Attends and participates in staff meetings and scheduled in-service training and/or additional educational opportunities as well as appropriate agency and interagency meetings
+ With Supervisor, responsible for discharge planning of residents
+ Acts as task supervisor when interns are assigned to Supported Housing Program
+ Responsible for Agency compliance requirements by grants through site visits, data collection, and tracking systems
+ Rotation of on-call duties
+ Responsible for other related activities
POSITION RELATIONSHIPS

POSITION REQUIREMENTS
• High School Diploma or Equivalency with experience in human services required
• Bachelor's Degree in Human Services or related field preferred
• Experience working with individuals with severe mental illness preferred
• Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle
• Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle
SHIFT: Sunday through Thursday (8:30 am to 4:30 pm), with an on-call availability
PAY RATE: $21.00 per hour
