    A Brief Overview

     

    The Behavioral Escalation Social Worker is a key member of the Behavioral Escalation Response Team (BERT), responsible for providing immediate intervention and support during behavioral health patient crises. This role involves crisis intervention, de-escalation, assessment, and collaboration with multidisciplinary teams to ensure the safety and well-being of patients and team members. The BERT Social Worker will also provide preventative screening, rounding, and care planning within the hospital environment.

    Essential Functions

    + Conducts Screening, Assessments, and Care Plans. - Conduct thorough assessments of patients at risk or experiencing escalated behavioral health actions to identify underlying issues, coping mechanisms, and immediate needs. - Provide emotional support to patients, helping them to manage their emotions and behaviors effectively.

    + Crisis Intervention and De-escalation - Resond promptly to behavioral escalation, providing immediate intervention and de-escalation techniques to manage and resolve the situation safely. - Use evidence-based practices to assess the situation and determine the most appropriate intervention strategies.

    + Collaboration and Communication - Work closely with the multidisciplinary team, including nurses, providers, security officers, unit leaders, and other social workers, to develop and implement individualized care plans for patients. - Facilitate communication between patients, families, and healthcare providers to ensure a coordianted approach to care.

    + Documentation and Reporting - Maintain accurate and timely documentation of all interventions, assessments, and care plans in accordance with Children's policies and regulatory requirements. - Report any incidents, concerns, or observations to appropriate leaders and other relevant parties.

    + Training and Education - Participate in the development and delivery of training programs for team members on behavioral health, crisis intervention, and de-escalation techniques. - Stay current with best practices and advancements in social work, behavioral health, and crisis intervention through continuous professional development.

    + Advocacy and Resource Connection - Advocate for the needs and rights of patients experiencing behavioral health crises, ensuring they receive the appropriate care and support. - Connect patients and families with community resources, support services, and follow-up care as needed.

    Education Qualifications

    + Master's Degree from an accredited college or university in Social Work Required

    Experience Qualifications

    + Minimum of two year's of supervised experience as a Social Worker Required and

    + Experience with care planning and de-escalation strategies Preferred and

    + Experience with children and adolescents Required

    Skills and Abilities

    + Requires knowledge of human behavior, social problems, counseling modalities, and community resources.

    + Ability to communicate effectively both verbally and in written form, relaying case information to patients/families, physicians, and other healthcare professionals, attorneys and law enforcement as needed.

    Licenses and Certifications

    + Current Provisional Master Social Worker in the state of Nebraska Upon Hire Required and

    + Current Provisional Mental Health Practitioner in the state of Nebraska Upon Hire Required

    + A valid driver's license and an acceptable motor vehicle record/driving record Upon Hire Required
