Case Management Supervisor
The Opportunity
FCA is seeking a Case Management Supervisor for our Expanded In-Home Services for the Elderly (EISEP) Program.
The Supervisor will be ensure the program delivers effective and quality services that meet all agency and contractual requirements by providing the supervision and oversight to the program staff and program records.
Responsibilities
+ Provide supervision to program staff including training, performance management and professional development.
+ Conduct chart audits to ensure timely, accurate and comprehensive client records.
+ Conduct regular case meetings with program staff to develop their skills, understanding and knowledge of clinical issues and problems related to the senior population.
+ Regularly contact clients to assess the quality of services being provided and to submit recommendations when needed.
+ Document compliance and or deficits and intake changes when required
+ Provide emergency service on an as-needed basis.
+ Attend all required OFA, county, and agency meetings.
+ Complete all required paperwork from agency, funder and any other regulatory agency and ensure it is submitted by the required deadline.
+ Attend all agency and funder required trainings including but not limited to compliance, diversity and supervisory training.
+ Additional responsibilities and projects as assigned to ensure the effective operation of the program.
Qualifications
Master Degree in Social Work or related field preferred.
Minimum of four years of experience with social case work, community social work, teaching, or community services work
or over one year of experience as a Case Manager.
Direct service to geriatric population is a plus.
Supervisory experience preferred.
Valid and clean driver's license required.
Additional Information
Salary: $64,350/Annually
Schedule: Full Time, 35 hours per week Monday-Friday
