    Infectious Diseases/Emergency Medicine Clinical Pharmacist

     

    Department: Pharmacy IP

     

    FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

     

    Shift: Day and Evening Rotation, Includes weekends

     

    Pay: $59.80 - $83.72 / hour. Pay is dependent on applicant's relevant experience

     

    This clinical pharmacist will have a primary focus in Antimicrobial Stewardship (AMS) and will cross cover the infectious diseases (ID) consult services and the emergency department. They will require independent decision-making, clinical judgement, and in-depth knowledge of general infectious diseases, transplant infectious diseases, and a mix of patient populations seen in the emergency department. This pharmacist will review, interpret and evaluate medication regimens to identify, prevent and resolve potential or actual medication-related problems. They will provide direct patient-care activities including initiating, modifying, and monitoring medication therapy through approved mechanisms such as therapeutic interchange, collaborative practice agreement, or other strategies. Shift timing will include day shifts, evening shifts, and weekend coverage.

    Minimum Requirements:

    + Bachelor's Degree in Pharmacy from an accredited school of Pharmacy.

    + State licensure as a Pharmacist and any relevant life support certification and/or relevant licensure/certification as determined at position level.

    + Completion of a PGY2 infectious diseases residency and experience in the emergency department preferred.

    Clinical Pharmacist:

    + Serve as a core member of the AMS Program, collaborating with ID physicians, microbiology, infection prevention, and other healthcare professionals.

    + Participate in daily stewardship clinical operations and program support including prospective audit and feedback, formulary management, guideline development, and education related to antimicrobial use.

    + Provide clinical pharmacy services to the inpatient general ID and transplant ID consult teams, including rounding, medication selection, dosing, monitoring, and transitions of care.

    + Support the Emergency Department through the provision of specialized clinical pharmacy services aimed at improving medication use and patient outcomes in this high-acuity setting.

    + Contribute to clinical research, quality improvement initiatives, and policy development related to AMS and ID pharmacotherapy.

    + Provide staff education and precept pharmacy students and residents.
