At Trinity Health At Home, we are driven by a shared mission to serve with compassion and excellence. We’re seeking a **Home Health Clinical Educator** to support and guide our nursing and therapy teams through orientation, ongoing training, and clinical excellence initiatives.
• **Key Responsibilities:**
+ Develop and deliver onboarding and continuing education programs
+ Serve as a mentor and clinical resource for new and current staff
+ Ensure compliance with CMS, accreditation, and internal standards
+ Evaluate clinical performance and support quality improvement
+ Lead initiatives to elevate care delivery and patient outcomes
+ Partner with leadership and staff to identify training needs
• **Qualifications:**
+ RN license in the State of Michigan required
+ Bachelor's degree in nursing or related field (master’s preferred)
+ Minimum 2 years of home health clinical experience
+ 1+ years of experience in a teaching, training, or educator role
+ Strong knowledge of Medicare regulations and OASIS preferred
+ Excellent communication, leadership, and teaching skills
+ Must have valid driver’s license and reliable transportation
