Using a person-centered approach Long Term Care Options Counselor will help consumers understand programs and services available to meet their long-term care needs. The LTC Options Counselor will provide assistance in-person, telephonically and virtually depending on the preference of the consumer. A long-term care options plan will be developed and shared with the consumer. The service may take place in one consultation or be a series of consultations depending on the needs of the consumers. LTC Options Counselors will be required to complete follow up calls, and documentation as required by the No Wrong Door grant guidelines. LTC Option Counselors will also be cross trained in Medicare through the SHIP program and will provide Medicare counseling on Medicare programs and services including Medicare Savings Programs and Extra Help programs to Medicare beneficiaries.
Duties Include:
+ Provide person-centered options counseling to consumers on programs and services available to meet their long-term care needs through in-person, telephonic or virtual appointments.
+ Develop long-term care plans and share with the consumer by mail, email or in-person depending on the preference of the individual. Complete required documentation of counseling in identified database.
+ Provide counseling on Medicare programs and coverage including Medicare Savings Programs, Extra Help and general Medicare questions and coverage. Complete required documentation in the STARS database.
+ Maintain person-centered options counseling certification and Other duties as assigned (to view the entire job description, please select the "download" button).
Qualifications
Minimum requirements include:
+ Bachelor Degree in Public health, Public Administration, or Social Work preferred.
+ Minimum 1-2 years experience in a non-profit Health and Human Service setting.
+ Excellent oral and written, interpersonal, and communication skills.
+ Ability to prioritize, work independently, and multi-task.
+ Experience working with older adults and adults with disabilities.
+ Effective time management skills including the ability to meet deadlines.
+ Strong technology skills, including intermediate Microsoft Office abilities.
