Essential Responsibilities:
Job Description
+ Under general supervision, assists with daily living activities
+ Observes and monitors behavior and charts observations and incidents.
+ Provides training of basic personal and social skills, simple academics, or work skills
+ Maintains a consistently high-quality level of care, serving as a role model
+ May assist with meal preparation, ensuring or encouraging group activity, providing transportation, and/or limited case management.
Roles and Responsibilities:
+ Arranges for needed medical care and provides needed services to assure that medical care is obtained.
+ Assists with the development of an individual service/treatment plan
+ Counsels and advises residents on behavioral problems, daily decision-making, and the resolution of minor problems.
+ Instructs and advise on daily living skills in one-on-one or group Instruction.
+ Observes and monitors behavior.
+ May provide corrective feedback on behavioral problems, daily decision-making, and the resolution of minor problems.
+ Participates in case of reviews, consulting with the team to evaluate the needs and plan for continuing services.
+ Prepares and maintains records of resident progress and services performed, reporting changes in resident condition to manager or supervisor.
+ Other duties as assigned.
Entry Qualifications :
Basic computer Skills.
Ability to communicate and interact with mental health professionals in a team environment, Ability to monitor and provide day-to-day assistance
Ability to observer assess and record symptoms, reaction, and progress.
Ability for understanding and follow safety procedures.
Documentation skills.
Valid driver’s license.
Preferred Qualifications:
Experience with or knowledge of individuals with intellectual/developmental disabilities; experience working successfully without daily contact with a supervisor.
Previous experience working in a residential home or healthcare facilities.
Understanding of basic first aid medical procedures.
Organizational skills and ability to coordinate the timely ordering/supervision of meds
Technical Competencies:
Ability to observe and monitor participants' behavior
Ability to assist and supervise participants with daily living activities
Ability to provide training and/or orientation services to participants
Ability to participate in supportive guidance, program planning meetings, consumer staffing and service training
Ability to complete evaluations to determine functional level of participants
This is hourly paid work performing health care assistance services. Positions in this job are established for less than 9 months or are scheduled for less than 30 hours per week and are not eligible for benefits if the position status is temporary (i.e., SCOA 513). Positions in this job are established for greater than 9 months and scheduled for greater than 30 hours per week if the position status is permanent labor (i.e., SCOA 512). Normally permanent labor positions are eligible for Health Insurance benefits.
Under direct supervision, learns to provide basic patient care and works with patients/clients to develop daily living skills. May take vital signs. Answers clients' call signals. Assists in administration of medications and treatments, takes vital signs, and/or draws blood or collects other bodily fluids, as directed by a physician or nurse. Bathes, grooms, shaves and dresses clients. Conducts activities for recreation and socialization. Ensures safety and security of clients. Feeds clients who are unable to feed themselves. Observes and documents patient data, behavior and activity. Prepares clients for treatment or examination. Provides client care by supplying and emptying bedpans, applying dressings and supervising exercise routines. Provides clients with help walking, exercising, and moving in and out of bed. Provides subprofessional occupational and/or physical therapy services to physically and/or developmentally disabled patients. Transports and/or accompanies clients to treatment units, using a wheelchair or stretcher. Turns and re-positions bedridden clients, alone or with assistance, to prevent bedsores. Receives additional training, as required, to gain full proficiency and experience in all areas

Experience working with individuals with mental illnesses.
