    Clinician, Clinical Time Transition Program

     

    Licensed Social Workers or Mental Health Counselors are eligible for $7500 signing bonus!

     

    MSW/MHC's with Limited Permit are eligible for $2500 signing bonus!

     

    Work related travel reimbursed at predetermined rates

     

    As a Clinician, CTTP, you will provide therapeutic support for youth aged 11-17 transitioning from a hospital setting back to their community. You will focus on ensuring a smooth reintegration by addressing mental health needs, strengthening support systems, and promoting long-term stability. You will report to the Director of the Hospital to Home (H2H) Program, and you will be working onsite with community travel.

    Responsibilities:

    + Conduct comprehensive mental health assessments to identify needs, strengths, and risk factors.

    + Develop individualized treatment and transition plans in collaboration with youth, families, and treatment team members.

    + Use trauma-informed and evidence-based approaches to support successful community reintegration.

    + Provide individual and family therapy to address emotional, behavioral, and adjustment challenges.

    + Facilitate skills training in areas such as emotional regulation, coping strategies, and social interactions.

    + Offer crisis intervention and stabilization support.

    + Work with families, schools, and community providers to coordinate care and ensure continuity of services.

    + Advocate for youth within educational, medical, and social service systems.

    + Connect youth and families with community resources, including outpatient therapy, mentoring programs, and vocational support.

    + Track progress through regular assessments and adjust treatment plans.

    + Maintain accurate clinical documentation in compliance with legal and ethical standards.

    + Participate in diverse team meetings to review cases and develop care strategies.

    + Provide guidance to families and caregivers on mental health, behavioral strategies, and available resources.

    + Collaborate with school staff and community agencies to promote youth success in academic and social settings.·

    Requirements and Education:

    + Master’s degree required

    + Limited Permit required; Professional licensure preferred: LMSW, LCSW, LMHC, or LMFT.

    + 1+ years of experience with complex mental health needs and co-occurring disorders

    + 1+ years of experience working within a clinical, hospital, or community-based setting preferred

    + 1+ years of experience with trauma-informed care, crisis intervention, and behavioral health treatment preferred.

    + Valid NYS driver's license required.

    + Must pass a physical.
