    Case Manager - RN

    Job Summary:

    We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our Care Coordination team. The ideal candidate will play a vital role in managing patient care plans, coordinating services, and ensuring high-quality outcomes across the continuum of care.

    Requirements:

    + Active Registered Nurse (RN)license in the state of Texas

    + Minimum 2 years of clinical nursing experience, preferably in case management, discharge planning, or care coordination

    + Strong understanding of healthcare systems, insurance plans, and community resources

    + Case Management Certification (CCM or ACM) preferred but not required

    Responsibilities:

    + Conduct comprehensive assessments of patients’ medical, psychosocial, and functional needs

    + Develop and implement individualized care plans in collaboration with interdisciplinary teams

    + Coordinate transitions of care, including discharge planning and referrals

    + Monitor patient progress and adjust care plans as needed

    + Advocate for patients and ensure access to appropriate services and support

    + Maintain accurate and timely documentation in compliance with regulatory standards

    Additional Qualifications:

    + Strong critical thinking and problem-solving skills

    + Excellent communication and teamwork abilities

    + Familiarity with electronic health records (EHR) and case management software

    + Knowledge of Medicare/Medicaid and commercial insurance guidelines

    Pay and Benefits

    The pay range for this position is $75000.00 - $85000.00/yr.

     

    benefits start 1st day of full month

    This is a fully onsite position in Austin,TX.
