    RN Care Manager

     

    We’re looking for a compassionate and organized Care Manager to join our team! In this hybrid role, you’ll coordinate medical and health services across various settings, including client homes, assisted living facilities, skilled nursing facilities (SNFs), hospitals, and clinics. Enjoy the flexibility of working both remotely and on-site with clients.

    Key Responsibilities:

    + Manage and oversee healthcare services and personnel

    + Develop and coordinate individualized care plans

    + Collaborate with physicians, care teams, and partners to ensure clients receive appropriate services

    + Uphold healthcare policies and advocate for exceptional client care

    Qualifications:

    + Experience in healthcare and case management

    + Strong interpersonal skills with a persuasive, empathetic, and caring approach

    + Excellent organizational, written, and verbal communication abilities

    + Wisconsin RN License

    Schedule:

    This is a Monday–Friday position with a flexible, self-managed schedule. You’ll spend approximately 50% of your time meeting with clients and the other 50% working remotely from home planning, coordinating care, and ensuring adherence to healthcare policies to deliver the highest quality of service.

    Pay and Benefits

    The pay range for this position is $35.00 - $38.00/hr.
