    Overview

     

    Starting Rate: $24.50

     

    The Senior Shared Living Coordinator manages, provides, and supervise high quality supports to the individuals and caregivers served by Shared Living program. They are responsible to train and mentor other Shared Living Coordinators.

    Additional Shift Details

     

    Monday-Friday 9am-5pm with some flexibility needed for evenings and weekends to meet families needs

    Responsibilities

    + Implement all aspects of the Shared Living program.

    + Act as program liaison to local DDS Area Office, Office of Quality Enhancement for Licensing and Certification, CARF and other Department personnel as delineated by Shared Living Director.

    + Act as liaison with other agencies involved in providing services to the individuals, we support such as day programs.

    + Ensure that the program meets and complies with all applicable DDS regulations: overseeing compliance of providers with mandated trainings.

    + Perform all responsibilities as identified in DDS regulations re "Placement Services”.

    + Collaborate with Recruitment and Transition Coordinator in recruitment, selection, and evaluation of Shared Living families.

    + Develop positive relationships with individuals, caregivers, families, and monitor/assess placements minimally on a monthly basis.

    + Train caregivers in all facets of their responsibilities.

    + Oversee Shared Living placements.

    + Support the development and growth of all individuals served in the program.

    + Collaborate and prepare for clinical case reviews with the Shared Living multidisciplinary team.

    + Ensure all program record keeping and documentation is accurate and up to date.

    + Carry a reduced caseload and implement all aspects of the program according to Advocates policies and procedures and state licensing and certification regulations.

    + Assist the Shared Living Director of Operations with internal audits to ensure overall program compliance.

    + Provide crisis intervention as needed and 24-hour on call rotation responsibility.

    + Assist with on-boarding and training of new Shared Living Coordinators.

    + Mentor Shared Living Coordinators

    + Oversee grievance policy for families and individuals.

    + Participate in state-wide shared living round table meetings and the yearly shared living conference.

    Qualifications

    + BS/BA or Associates Degree in related field with at least 2 years of experience working with individuals with developmental disabilities. 1-2 years’ experience working in Shared Living program.

    + Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

    + Ability to communicate effectively verbally and in writing and able to use good judgment.

    + High energy level, superior interpersonal skills and ability to function in a team atmosphere.

    + Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
