Praxis Health is seeking a dynamic and compassionateBehavioral Health Consultant (BHC)to join our integrated care teams in beautiful Bend, Oregon. This is an exciting opportunity to be part of a fast-paced, team-based environment that truly embodies whole-person care.
Unlike traditional outpatient therapy, our model follows thePrimary Care Behavioral Health (PCBH)approach—consultative, collaborative, and focused on brief, functional interventions that help patients improve overall health outcomes.

More About The Role:
+ Deliverbrief (30-minute), evidence-based interventionsfor a broad range of behavioral and health concerns—depression, anxiety, chronic illness, stress management, lifestyle changes, and more
+ Conductfunctional assessmentsthat support real-time decision-making in a primary care context
+ Partner directly with primary care providers, offering consultative input to improve patient outcomes
+ See patients1–4 times on average, without the burden of maintaining a traditional therapy caseload
+ Support bothin-person careand occasional telehealth visits throughout your clinic day
+ Participate inteam huddles and care planning, contributing to a truly integrated treatment model
+ Have the opportunity todevelop psychoeducational groups and wellness-focused initiatives

Minimum Qualifications
+ Educational Qualifications: Master’s degree in Social Work, Psychology, Counseling, or a closely related field.
+ Licensure Requirements: Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT).
+ Experience working in an integrated care setting preferred but not required.
+ Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy.
+ Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers.
+ Comfortable with using telehealth technology for remote patient care.
+ Flexible and adaptable to changing work environments and patient populations.
+ Commitment to providing culturally competent and trauma-informed care.
+ Will attend work regularly and on time, as scheduled.

General Physical Requirements
+ Mobility: Ability to move within a clinical setting to engage with patients and team members.
+ Sitting: Prolonged periods of sitting for patient consultations and administrative tasks.
+ Manual Dexterity: Frequent use of a computer, keyboard, and other office equipment for documentation.
+ Communication: Strong verbal and auditory abilities to effectively interact with patients and colleagues.
+ Visual Acuity: Ability to read and interpret patient records and electronic documentation.
