Social Worker for Assisted Living

Collingswood Manor, a UMC Community in Camden County, is a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking a Social Worker .
Requirements for a Social Worker:
+ Bachelor's degree in Social Work (BSW) or Human Service Field
+ Bachelor's degree in Social Work (BSW), and Certified Social Worker license with the State of New Jersey (CSW) perferred
+ Proficiency in a clinical software system.
+ Understanding of the MDS, care plan and admission and discharge processes.
+ One year Healthcare/Long Term Care experience. Proficiency in multiple clinical software systems preferred
+ Five years' experience in the Healthcare/Long Term Care setting preferred
+ Certified Social Worker license with the State of New Jersey (CSW).
+ ACSW preferred
Responsibilities for a Social Worker:
+ Develop, maintain and utilize a listing of current community resources.
+ Advocate for the resident and their family.
+ Understands and meets all requirements for social service documentation, i.e. interdisciplinary care plan; d/c notes; advanced directives; resident rights, etc.
+ Prevents and addresses resident abuse.
+ Educate associates regarding residents' rights including how to recognize and prevent abuse, neglect and mistreatment, resident dignity, independence and quality of life - as outlined in the Resident Rights.
+ Provide the residents, families, significant others and associates with communications regarding care conferences such as the date and the time of the meeting.
