    The Pharmacy Department at Union Health is looking for team members for the Terre Haute campus.     The Inpatient Clinical Pharmacist plays an integral role by managing the use of medications in hospital with goals include the selection, prescription, procuration, delivery, administration, and review of medications to optimize patient outcomes and assures that services are provided within professional standards and organizational policies to assist patients who lean on the Pharmacy team for answers.

    Clinical Pharmacist Essential Job Duties:

    + Performs drug information functions as necessary to meet the needs of the patients, medical, nursing, ancillary departments, and pharmacy staff.

    + Observes and complies with health system policies and regulations.

    + Observes and complies with state and national pharmacy law

    + Performs special projects at request of pharmacy administration

    + Performs all other duties as assigned by pharmacy administration.

    + Verifies medication orders

    + Adjust medication orders per hospital approved policies (i.e. renal dose protocol, therapeutic interchanges, IV to PO conversion, etc.)

    + Verifies/checks IVPBs, LVP, PN, oral dosage forms, topicals, vials, and other injectables

    Education, Training & Experience :

    + Advanced Pharmacy Degree (Pharm.D.)

    + Board Certification preferred in the first year of eligibility.

    + One to two years experience in ASHP accredited residency or equivalent experience preferred.

    + Licensed or eligible for licensure in the State of Indiana. Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position. Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.

    + Must have good verbal and written communication skills.

    + Maintain continuing education per state licensing requirements.

    + Flexible work schedule to meet needs of the department.

    + Excellent grasp of standard written and verbal English
