    In Home Counselor

     

    The Opportunity

     

    Family and Children’s Association (FCA) is a Full-Time In-Home Counselor for our Children's Mental Health Division. The In-Home Counselor provides individual, family, and group services under Community Psychiatric Supports and Treatment to children and their families who are diagnosed or at risk-of being diagnosed with a mental health disorder. Services are provided in the client's home and/or community and work to address challenges associated with a behavioral health need.

    Responsibilities

    + Deliver in-person, community- and home-based services to children and their families to support behavioral and emotional functioning.

    + Prepare and submit timely progress notes and treatment plans in accordance with state and Managed Care Organization (MCO) guidelines

    + Collaborate with Care Managers and other service providers to ensure coordinated care.

    + Ensure all documentation and service delivery comply with agency standards, funding requirements, and the Compliance Plan.

    + Utilize a strength-based, family-centered approach in all service interactions.

    + Meet all established deadlines and maintain accurate, up-to-date records.

    + Perform additional duties as assigned to support the effective operation of the program.

    Qualifications

    + Bachelor’s degree in Social Work or related field with a minimum of three years’ relevant experience, or a Master’s degree with at least one year of experience working with children with emotional impairments.

    + Demonstrated ability to work effectively in a fast-paced environment with strict deadlines.

    + Preferred experience includes delivering Medicaid billable services and working with children and adolescents, particularly those with mental health diagnoses.

    + Strong knowledge of community resources and mental health conditions.

    + Valid New York State driver’s license with a safe and responsible driving record.
