Non-Medicaid Care Manager

The Opportunity

Family and Children’s Association (FCA) is seeking a Care Manager to provide support, service coordination, and linkage to resources for children and youth ages 5 to 21 with a serious emotional disturbance.

Responsibilities

+ Provide consistent case management through at least two contacts per month, using a strength-based, family-driven approach.

+ Complete and submit all required documentation (assessments, care plans, progress notes, billing, etc.) in compliance with agency and regulatory standards.

+ Conduct ongoing assessments of child and family strengths, needs, and progress toward goals, adjusting services as necessary.

+ Coordinate care by maintaining contact with families and providers and facilitating interdisciplinary team meetings.

+ Support program operations by assisting with specialized processes (e.g., C-YES for Non-Medicaid cases), attending staff meetings, and completing additional assigned tasks.

Qualifications

+ Bachelor's Degree required

+ Minimum of 2 years of related experience

+ Intermediate Computer skills

+ Excellent verbal and written communication skills.

+ Valid NYS Driver's License

Compensation
