The Opportunity
Family and Children’s Association (FCA) is seeking a Per Diem Care Manager to join our Home and Community-Based Services (HCBS) Program.
The Care Manager is responsible for coordinating and connecting youth and their families with community-based resources and services that support stability and help maintain the youth in the home. This role also involves ongoing collaboration with the youth’s treatment team to ensure continuity of care and progress toward identified goals.
Responsibilities
+ Provide case management services to youth across Nassau County and Western Suffolk, through both in-person and telephonic visits.
+ Conduct ongoing assessments to identify youth and family strengths, needs, and progress toward care plan goals.
+ Link families to appropriate services, supports, and resources as outlined in individualized care plans.
+ Complete and submit all required documentation—including assessments, care plans, progress notes, billing, and chart updates—on time and in compliance with agency and regulatory standards.
+ Maintain regular communication with internal and external service providers, and keep supervisors informed of case status and any safety concerns, including potential CPS reports.
+ Participate in required trainings and team meetings, and deliver services using a strength-based, family-driven approach.
Qualifications
+ Bachelor's degree in a human service-related field.
+ Minimum of 2 years relevant experience.
+ Excellent verbal and written communication skills.
+ Proficient computer skills with MS Office.
+ Valid and clean Driver's License.
