Cleveland Clinic is seeking a dynamic, experienced professional to lead safety, quality and patient experience for our Center for Connected Care. This role will make an impact on healthcare across the organization and within the communities we serve, ensuring we provide safe, top-quality care within the areas of Home Care, Hospice, Home Infusion, Home Respiratory Therapy, Medical Care at Home and SNF Connected Care.
The Director of Safety, Quality and Patient Experience (SQPE) directs the safety, quality and patient experience within the Center for Connected Care. This leader acts as a member of the local leadership team and supports unique challenges. This leader models Cleveland Clinic Core Values and behavioral standards, ensures compliance with reporting, accreditation, and governance requirements. Additionally, this leader enables the STEEEP (Safe, Timely, Effective, Efficient, Equitable and Patient-Centered) focus on SQPE.
This caregiver also leads the SQPE department to ensure consistency in application of assurance and improvement processes, collaborates with subject matter expert leaders in nursing quality, accreditation, EHS (Environmental Health and Safety), Radiation Safety, Safety, High Reliability, Infection Prevention, etc., to drive the organization to a consistent approach to being the best place to receive care anywhere.
A caregiver in this role works Monday-Friday, from 8 a.m. -- 5 p.m., on-site at our Business Operations Center.
A caregiver who excels in this role will:
+ Collaborate with leadership and support teams to develop and maintain an integrated Quality Plan to achieve quality, safety, patient experience and compliance goals.
+ Act as an accountable partner for all clinical areas within areas of responsibility.
+ Communicate priorities to stakeholders at all levels and drive performance on OKRs related to safety, quality, patient experience and compliance.
+ Support committees for quality improvement.
+ Drive continuous improvement activity around quality, safety, patient experience and compliance priorities and coordinate improvement projects.
+ Ensure continuous survey readiness.
+ Act as the local leader to reduce infections.
+ Measure and analyze data reflecting processes and outcomes prioritized by the Center for Connected Care and the Clinical and Operational Improvement Leadership Team. Act as the local expert in safety and quality performance.
+ Facilitate dashboard reviews, data analysis and decision-making from data. Identify and address root causes to lead to sustainable improvement.
+ Act as a champion for continuous improvement, utilizing the Cleveland Clinic Improvement Model.
+ Coach others to improve organizational capacity for improvement.
+ Manage the budget for the local quality department as a steward of limited resources.
+ Support enterprise improvement projects as necessary.
+ Develop relationships with skilled nursing facilities, home care agencies and other groups that support the patients served.
+ Participate in hospital readmission reduction meetings.
+ Be responsible for monitoring and reporting on compliance within the Center for Connected Care.
+ Be responsible for workplace violence and safety initiatives for the Center for Connected Care.
+ Lead critical incident reviews for the Center for Connected Care.
+ Be responsible for coordinating policy and procedure updates within the Center for Connected Care.
+ Act as a liaison between the Center for Connected Care and governing bodies.
+ Present on quality performance and goals.
+ Ensure productivity, performance and engagement on local Clinical and Operational Improvement team members, regardless of reporting relationships.
+ Manage Safety, Quality and Patient Experience across multiple geographic locations within the scope of the Center for Connected Care.
Minimum qualifications for the ideal future caregiver include:
+ Bachelor's degree in nursing, healthcare administration, business, engineering or related field
+ Certified Professional in Healthcare Quality (CPHQ) required within one year of hire
+ Minimum of seven years of process improvement or quality management experience
+ Progressive quality and performance improvement research, leadership, supervision or project team management
Preferred qualifications for the ideal future caregiver include:
+ Master's degree
+ Certified Patient Safety Professional (CPPS), Healthcare Accreditation Certification Program (HACP) or Certified Joint Commission Professional (CJCP), Process Improvement Certification (Lean, Six Sigma, etc.), Certified in Infection Control (CIC), Certified Patient Experience Professional (CPXP)
+ Healthcare experience specifically in Home Care or Hospice setting

Physical Requirements:
+ Ability to communicate and exchange accurate information.
+ Ability to perform work in a stationary position for extended periods.
+ Ability to work with physical records or operate a computer or other office equipment.
+ In some locations, ability to travel throughout the hospital system.
+ In some locations ability to move up to 25 lbs.
Personal Protective Equipment:
+ Follow standard precautions using personal protective equipment as required.
Pay Range
Minimum hourly: $53.90
Maximum hourly: $82.19
The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set, and education.
