+ GENERAL SUMMARY

+ Care Connection for Children is a community-based program for Children and Youth with Special Health Care Needs (CYSHCN). This community-based program ensures that services comply with the contract requirements set by the Virginia Department of Health. The Program Manager is responsible for ensuring the program achieves its objectives, complies with regulations, and delivers coordinated care for individuals in need of comprehensive care coordination services.

The role involves collaborating with program staff, families, healthcare providers, and community organizations to provide high-quality, family-centered care. The Program Manager acts as the liaison between CHKD and the Virginia Department of Health, which oversees the statewide Care Connection for Children network. Reports to department leadership.

+ ESSENTIAL DUTIES AND RESPONSIBILITIES

+ Program Oversight & Management:

+ Lead the design, execution, and daily operations of the program.

+ Ensure the program aligns with contract requirements and organizational policies.

+ Manage the program's budget, ensuring efficient use of resources within allocated limits.

+ Track and assess program performance, focusing on key performance indicators (KPIs), outcomes, and service delivery metrics.

+ Secure long-term program viability through funding strategies, including grant management and partnerships.

+ Staff Management & Training:

+ Lead the design, execution, and daily operations of the program.

+ Ensure the program aligns with contract requirements and organizational policies.

+ Manage the program's budget, ensuring efficient use of resources within allocated limits.

+ Track and assess program performance, focusing on key performance indicators (KPIs), outcomes, and service delivery metrics.

+ Care Coordination:

+ Track and assess program performance, focusing on key performance indicators (KPIs), outcomes, and service delivery metrics.

+ Secure long-term program viability through funding strategies, including grant management and partnerships.

+ Data & Reporting:

+ Provide regular updates on program outcomes, funding utilization, and performance metrics to the Virginia Department of Health and CHKD leadership, as required.

+ Ensure compliance with reporting and documentation requirements as outlined by contract funding expectations.

+ Performs all other duties as assigned.

+ LICENSES AND/OR CERTIFICATIONS

+ Required Licenses and/or Certifications

+ + Registered Nurse or MSW required.

+ + Certification as case manager by accredited case management entity required.

+ Preferred Licenses and/or Certifications

+ + None preferred.

+ MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

+ Required Education and Experience

+ Master’s degree in nursing, healthcare management, social work, public health, or related field required.

+ Minimum of 5 years of experience in managing or coordinating care for children and youth with special health care needs (CYSHCN) required.

+ Experience in data collection, program evaluation, and quality improvement initiatives.

+ Preferred Education and Experience

+ None preferred.

+ Required Knowledge, Skills and Abilities

+ + Strong knowledge of community resources, health care systems, and case management for CYSHCN.

+ + Knowledge of local, state, and federal regulations related to CYSHCN and care coordination programs.

+ + Excellent communication and interpersonal skills, with the ability to work effectively with families, healthcare providers, and community organizations.

+ + Proficiency in care coordination software and Microsoft Office Suite.

+ + Knowledge of quality improvement methods and methodology in data collection and program evaluation.

+ + Ability to function at a high level of independence in performance role and manage multiple tasks and assignments

+ + Ability to work in a fast-paced, dynamic environment with attention to detail.

+ + Cultural competence in working with diverse populations.

+ + Strong organizational and problem-solving abilities.

+ WORKING CONDITIONS

+ Normal office environment with little exposure to excessive noise, dust, temperature and the like. Also, community based may require home and agency visits, local travel.
