Unity House of Troy is seeking a skilled mental health RN for either full-time or part-time hours for the Crisis Residence located in Troy NY. You could make a difference in someone's life!

Flexible hours on evenings and weekends

$5000 sign on for full-time ( sign on bonus pro-rated for part-time)

The Crisis Residence offers a safe place for the stabilization of mental health symptoms and a range of person-centered trauma informed services to support participants. This is a short term stay, 6 bed facility serves participants for up to 28 days as they prepare to transition back to the community.

You will make significant impact on advancing this comprehensive specialty care residence and provide meaningful support to the team.

Responsibilities:

+ Conduct mental and physical health assessments.

+ Help create individualized participate service plans.

+ Participate in care coordination.

+ Work as part of a multi-disciplinary team to meet participants needs.

+ Provide linkages to relevant medical services and preventative care.

+ Promote overall wellness and self-management.

+ Provide accompaniment, support and advocacy as needed.

+ Participate in rotating on-call system.

+ Promote a culture of inclusion and belonging.

Requirements:

Current, unrestricted NYS Licensed Registered Nurse. Must be current with CPR and First Aid certification. At least one-year experience working in mental health/substance abuse settings. Clean and valid NYS driver license. Must be able to work independently and as a team. Proficient with computers.
