Ultimate Care NY is seeking dedicated and compassionate Home Health Aides to join our team in providing exceptional care to our patients in the 10703 area. As a Home Health Aide, you will play a crucial role in helping our clients maintain their independence and dignity while receiving the essential support they need. You will work closely with individuals and their families to provide a nurturing environment that promotes health and well-being. Responsibilities

+ Assist clients with daily living activities including bathing, dressing, grooming, and meal preparation.

+ Monitor and record patient vital signs and report any changes to the healthcare team.

+ Provide companionship and emotional support to clients and their families.

+ Assist with mobility and transportation of clients to and from appointments or daily activities.

+ Administer basic medication reminders and assist with adherence to prescribed medication schedules.

+ Maintain a clean and safe environment for clients at home, ensuring hygiene and safety standards are met.

+ Communicate effectively with clients, family members, and healthcare personnel to ensure coordinated care.

+ High school diploma or equivalent preferred.

+ Valid Home Health Aide certification from an accredited program.

+ Previous experience in home health care or similar caregiving roles is a plus.

+ Strong interpersonal and communication skills to interact with clients and their families.

+ Ability to follow instructions and care plans as directed by healthcare professionals.

+ Compassionate and patient attitude towards clients, demonstrating empathy and respect.

+ Physical ability to perform tasks needed to assist clients, including lifting, bending, and mobility support.
