Duties and Responsibilities

+ Participates as part of the clinical team, providing assistance with managing intake, triage and urgent care/crisis systems and consultation regarding community referrals.

+ In coordination with the Wellness Center Counseling Center staff, the Behavioral Concerns Team and Care Team staff, and/or other campus personnel, assists and supports students in accessing services from community resources for outpatient mental health care. Examples include scheduling phone appointments for students who need referral information, meeting with students who need referral information, meeting with students to overcome barriers to accessing community resources, and providing care management for students who may need multiple services across campus.

+ Develops and implements systems for follow-up with students who have been referred to community resources (psychotherapists, psychiatrists, clinics, intensive outpatient treatment programs). Follows up with external providers to ensure that clients are appropriately assessed, successfully referred, and compliant with treatment recommendations. In addition, as needed, the care manager supports medical staff in connecting students to community providers for medical specialties, similarly assisting students in accessing affordable community care through insurance network providers.

+ Assesses/analyzes the insurance benefits and financial resources available for mental health treatment for identified students/clients (including advocating for low cost services when needed).

+ Assists students who have been hospitalized for mental or physical illness, to transition successfully back to Loyola by assisting with discharge planning, coordinating follow-up care, and facilitating students’ access to appropriate services on- and off-campus. Provides care management of these students consistent with best practices for college students who have experienced significant episodes of mental illness.

+ Assists and collaborates with counseling staff in treatment and disposition planning for students with complex mental health needs or physical illness, particularly for those students who are frequent users of urgent care appointments. Also assists medical staff with referrals for students seeking long term, or specialty medical support in the community.

+ Consults with parents, faculty, school administrators, student development staff when appropriate and with appropriate authorization in order to ensure quality care to students

+ Maintains appropriate records and documentation of care management activities, in compliance with law, ethics and best practices.

+ Cultivates strong liaison relationships with off-campus mental health providers, private practitioners, hospitals and inpatient units, and other community partners.

+ Maintains a comprehensive database for mental health providers in the Chicagoland area and will assist students in finding mental health care near their homes.

+ Attends and participates in Wellness Center all-staff and clinical meetings, committee meetings by assignment, and other staff meetings and training activities.

+ Assists with mental health phone daytime coverage.

+ Might carry small brief counseling case load during times of peak student demand, as caseload volume allows.

+ Pursues professional development in service of the responsibilities of the position and profession.

+ Performs other duties as assigned

Minimum Education and/or Work Experience

+ Master’s degree in social work, counseling or related mental health discipline.

Qualifications

+ Must have master’s degree in social work, counseling or related mental health discipline.

Certificates/Credentials/Licenses

+ Current valid licensure in area of mental health specialization (e.g., LSW , LCSW , LCPC ) is preferred.

+ Applicants who are eligible for licensure but require supervised clinical hours for licensure may be considered and are encouraged to apply.

Computer Skills

Proficiency in basic word processing;

Experience with using an electronic medical record system is preferred, must have ability to learn this skill.
