    Minimum Requirements:

    Licensed to practice medicine in accordance with Chapter 458, Florida Statutes* plus four years of appropriate experience. Completion of a residency program may substitute for the required experience.

    Preferred Qualifications:

    + Certification/Licensure in the state of Florida

    + Federal DEA Certification.

    + Medical Degree – M.D., D.O., or foreign equivalent.

    + Board certified in Family Medicine.

    + Completion of an accredited residency program.
