    The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) applies advanced education and clinical competencies to achieve optimal patient outcomes. The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works on problems of diverse scope and complexity ranging from moderate to substantial. Nurse Practitioner, Virtual Care (VSP/PT) provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. This position will focus on Quality Measures as well as some assistance with acute access.

     

    The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Prescribes medication, treats patients, diagnoses illnesses, and provides treatment. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks and works under minimal supervision. Uses independent judgment requiring analysis of variable factors and determining the best course of action.

    Responsibilities:

    The Acute Access Nurse Practitioner, Virtual Care (VSP/PT) works in our team-based care environment.  Ideally, the Acute Access Nurse Practitioner, Virtual Care (VSP/PT) will provide care for patients via virtual visits.  Visit types conducted will focus on Medicare Wellness visits and acute same day visits.

    Patient Care:

    + The Acute AccessNurse Practitioner,Virtual Care(VSP/PT) will dedicate their clinical efforts to improving access for the patients we serve.

    + Evaluates and treats center patients in accordance with standards of care.

    + Follows level of medical care and quality for patients and monitors care using available data and chart reviews.

    + Acts as an active participant and lead with the care team through daily huddles.

    + Helps Associate Medical Director (AMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.

    + Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.

    +  Follows policy and protocol defined by Clinical Leadership.

    + Meets with AMD about quality of care, review of outcome data, policy, procedure, and records issues.

    + Assures personal compliance with licensing, certification, and accrediting bodies.

    Required Qualifications

    + Master of Science in Nursing

    + Successful completion of a Nurse Practitioner program with Board Certification

    + Active, unrestricted Nurse Practitioner license in South Carolina

    + Minimum of 2 years directly applicable experience preferred.

    + Excellent communication skills written and verbal, demonstrate a high level of skill with interpersonal relationships and communications with all colleagues; fully engaged in the concept of “Integrated team-based care”.

    + Willingness and ability to learn/adapt to practice in a value-based care setting.

    + Superior patient/customer service.

    + Basic computer skills, including email and EMR.

    + Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director.

    + Active, unrestricted Nurse Practitioner license in South Carolina

    + Must be passionate about contributing to an organization focused on continuously improving consumer experiences

    Preferred Qualifications

    + Active and unrestricted DEA license

    + Medicare Provider Number/ Medicaid Provider Number

    + Knowledge of Medicare guidelines and coverage.

    + Knowledge of HEDIS quality indicators.

    + Experience working in value-based care settings

    + Telehealth experience

    + IHWA experience

    + Bilingual in English and Spanish

    Work-At-Home Requirements

    To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:

    - At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested

     

    - Satellite, cellular and microwave connection can be used only if approved by leadership.

     

    - Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.

     

    - Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.

     

    - Work from a dedicated space lacking ongoing interruptions to protect member PHI/HIPAA information.

    The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

     

    $115,200 - $158,400 per year
