The Women’s Health Nurse Care Manager provides holistic nursing care to patients across assigned PHC clinics and serves as a patient advocate and resource through the continuum of care. Demonstrates iCare values in all interactions.
Note: This position will regularly travel to work at clinics in Ames, Des Moines, and Marshalltown. The schedule is anticipated to work Monday - Friday 8 AM - 5 PM which will include three days per week in Des Moines, one day per week in Ames, and one day per week in Marshalltown but could change based on patient needs.
What You Will Do
+ Manages new OB patient intakes and histories, completes nursing assessments and comprehensive medication reconciliation, supports the risk stratification process, and consults with medical providers on care plans for ongoing prenatal and delivery services.
+ Completes record requests, chart preparation, and pre-visit planning.
+ Collaborates with appropriate internal resources to coordinate presumptive Medicaid, other financial assistance, and entitlement applications.
+ Administers non-stress testing in coordination with medical providers.
+ Triages patients in-person or on the phone according to established protocol.
+ Supports patients through their care plans and utilizes the care team and resources to empower them, including holistic counseling, problem solving “barriers to care” and coaching as needed regarding the patient care plan, treatment, medication adherence, healthy lifestyle choices, and self-help behaviors.
+ Documents patient care accurately and timely in the electronic health record.
+ Participates in training, orientation, competency assessment, and performance evaluations of clinical support staff, students, and apprentices. Serves as a resource to staff and promotes effective collaboration.
+ Assists clinical support staff as needed, including but not limited to, facilitating patient rooming and intake, obtaining vital signs; performing point-of-care, lab, and diagnostic testing per orders; administering medications, immunizations, and treatments per orders.
+ Performs quality assurance reviews for life safety controls/tasks and duties assigned to clinical support staff.
+ Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures.
Qualifications You Need to Bring
Required
+ Completion of an accredited nursing program accepted by the Iowa Board of Nursing for licensure.
+ A minimum of 2 years of RN experience. Note: LPN experience may be partially substituted for experience at leadership discretion for internal candidates.
+ Effective nursing skills and knowledge of nursing care regulations and practices established by the Iowa Board of Nursing with ability to apply universal precautions in patient care.
+ Excellent interpersonal, teamwork and collaboration skills with ability to develop and maintain effective working relationships and take a team approach to care.
+ Effective written and verbal communication skills.
+ Effective leadership skills.
+ Excellent customer service skills.
+ Basic computer skills with proficiency using Microsoft Office applications, electronic health record and other database systems.
+ Strong organization and prioritization skills and is detail oriented.
+ Professionalism skills with ability to maintain composure in all situations, demonstrate support and interact empathetically with diverse groups of individuals.
+ Effective problem-solving skills.
+ Licenses & Certifications: Current RN license recognized by the State of Iowa Board of Nursing; *Current Basic Life Support (BLS) for Healthcare Providers; *Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa
* Must be obtained within the Introductory Period if not current.
Preferred
+ Bachelor’s degree in nursing.
+ Work experience in a community health center or obstetrical care.
+ Experience in a leadership role.
+ Effective verbal and written communication skills in Spanish.
