    Care Coordinator (Case Management) Liaison

    We are seeking a dedicated and compassionate Care Coordinator (Case Management) Liaison to join our team in supporting individuals experiencing homelessness. This role is responsible for providing direct case management and supportive services to emergency shelter clients, while serving as a key connection between clients and the Trenton/Mercer Continuum of Care (CoC) Coordinated Entry and Assessment System (CEASe).About the Rescue Mission of Trenton:Located in Trenton New Jersey we serve men and women who have no place else to turn for shelter, clothing, treatment and food. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. Our clients are assisted by staff with treatment options, medical help, job placement, financial planning, education, case management and mental health treatment. This position is an important part of helping our clients navigate independent living.Qualifications:

     

    + Education: Bachelor’s degree required; Master’s in Social Work or related field preferred.

    + Experience: Minimum of 3 years in case management or related human services role.

    + Certification: Clinical Social Worker (CSW) preferred.

    + Skills: Strong interpersonal, critical thinking, and documentation skills; understanding of addiction and recovery support.

    + Technical: Proficiency in Microsoft Office (Word, Excel); experience with HMIS a plus.

    + Other: Valid driver’s license required.

    Key Responsibilities:

    + Serve as a liaison between clients, emergency shelters, day centers, and CoC-CEASe providers.

    + Provide direct care coordination, including referrals, housing support, and financial assistance.

    + Develop and monitor individualized service and after-care plans.

    + Maintain accurate and timely documentation in HMIS and client records.

    + Collaborate with internal staff and external partners to ensure comprehensive client support.

    + Advocate for clients at both individual and community levels.

    + Participate in safety procedures, staff meetings, and ongoing professional development.

     

    Salary: $47,528.32 plus a comprehensive benefit and training package

     

    Work Schedule: Tuesday, Friday and Saturday 9am to 5pm and Wednesday and Thursday 11am – 7pm.The Rescue Mission of Trenton is committed to ensuring that all current and prospective employees are afforded equal opportunities and treatment and a harassment-free work environment.
