The Penn Transplant Institute has been a pioneer in organ transplantation for nearly fifty years. Penn's multidisciplinary team leads the region in the total number of transplants performed and is ranked among the top 10 multi-organ transplant centers in the country. Our surgeons perform over 200 kidney transplants every year, making Penn's kidney transplant program one of the largest and most experienced kidney transplant program in the region.
Position Description:
The transplant advanced practice provider (APP) provides and maximizes the quality of care to patients referred to the Penn Transplant Program with end-stage kidney and/or pancreas failure. The transplant APP provides direction in the management of patient care throughout all phases of the transplant process: organ failure, evaluation, transplantation, and follow-up care. The transplant APP performs a systematic assessment, evaluation and develops a plan of care for patients and their significant others; develops a written, continuous record for each patient; implements identified medical and nursing interventions; and evaluates outcomes. The transplant APP serves as an integral member of the transplant team in meeting the needs of the transplant patient and ensuring continuity of care throughout the transplant process. This is an excellent opportunity for an APP to provide longitudinal care to a complex population and be a well-respected member of the team.
Minimum Requirements:
Licensed to practice in the Commonwealth of Pennsylvania, M.S.N., and certification as a Nurse Practitioner or Masters Physician Assistant. (Family, Adult, or Acute Care certification)
Prior to employment, the AP must be reviewed by the Credentials Committee and approved by the Medical Board
Additional Information:
This position is Monday through Friday 40 hours per week with rotational, shared on-call duties.
Hours may be adjusted to meet the operational needs of the department.
