Job Summary:
As a Case Manager for Healthy Families, you will provide in-home education and supportive services to new and expectant parents in Bergen and Hudson Counties, especially those families overburdened by stressors placing them at risk of child abuse and neglect. In this role, you will provide prenatal/post-natal screenings, assessment, and long-term direct prevention services including case management, intensive home visits, telephone calls, and monthly support groups for parents referred by the board of social services.
What You Will Do:
+ Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness
+ Engage in supportive counseling, crisis intervention, and offer on-site as well as outreach services (including transport of consumers)
+ Communicate observations and information about the consumer in the consumer's service record and to team members
+ Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
+ Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans and participate in quality assurance activities.
+ Provide input into formulating consumer-centered care plans in collaboration with team members.
+ Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collect consumer information and reflect all demographic data.
What WeAre Looking For:
+ High School Diploma or equivalent
+ One year of experience in a mental health-related field.
+ Must be able to communicate with clients in their native language (Spanish)
+ A valid driver's license and must be insurable by the agency's automobile insurance
