Description
Prevention Case Planner provides intensive home- based services utilizing evidenced-based practices in partnership with enrolled individuals and families with children at-risk of placement in the foster care system. Tasks include individual and family therapy, psychosocial assessments, collaboration with an interdisciplinary team, providing psychoeducation to children and families, casework counseling, crisis intervention and linkage to community resources. Case Planner maintains records in accordance with regulatory requirements.
Duties & Responsibilities:
•Provides indicated direct casework services to client on assigned caseloads under professional social work supervision.
•Responsible for submitting written reports for the client record as required by the agency according to time frames set by the program.
•Responsible for preparing required external accountability reports and forms on case progress for assigned cases to local departments of social services.
•Communicates with other team and staff members to ensure integrated service delivery to the child/family.
•Serves as a primary provider of appropriate direct services to the child's family based on need assessment and the treatment plan for the child/family. Arranges home visits for the child.
•Keeps other treatment team members informed of general progress of the child in relation to the family and of all significant developments regarding the family.
•Provides indicated after-care services including direct services and linkage to other supportive or needed services.
•Provides family therapy to assigned cases.
•Able to maintain a caseload between 8-10 cases.
•Able to operate agency motor vehicles and possess a valid New York State driver’s license with a motor vehicle record that is acceptable to the agency.
•Must be able to climb and descend stairs without assistance in settings not in compliance with or under the mandates of the Americans with Disability Act.

Requirements
+ Bachelor’s Degree.
+ Background in home visiting programming.
+ Good communication and counseling skills with individuals.
+ Knowledge of community and public social service agencies.
Salary Description
$55,016-$55,016/year
