Description:
Manage patients on anticoagulants by interpreting treatment goal and adjusting anticoagulants as necessary in Mercy Health's anticoagulation clinic.
Collaborate with other healthcare providers as well as providing direct patient care.
Meet with patients individually to provide extensive drug and disease state education targeting complex disease states such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis.
Work with patients to improve adherence and evaluate medication regimens.
Assist in supervision of pharmacy technicians

What the Pharmacist, Ambulatory will need:
+ BS degree in Pharmacy.
+ Preferred: Advanced degrees (i.e., Doctorate of Pharmacy)
+ Licensed to practice in the State of Michigan.
+ Preferred: PGY1 or PGY2 ASHP accredited residency
+ Preferred: 2-3 years in hospital or ambulatory setting
What the Pharmacist, Ambulatory will do:
