Summary The General Caregiver Support Program Coordinator is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence-based services and interventions: program development; caregiver, Veteran, and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities and evaluation/consultation. Responsibilities Total Rewards of a Allied Health Professional The coordinator will function in support of caregivers, Veterans, and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services. This includes development of a strategic plan for expansion of the General Caregiver Support Program and conducting frequent needs assessments to implement education, wellness, and support groups. Duties will include but are not limited to: Develop and coordinate all details for the Program of General Caregiver Support Services (PGCSS). Meet and exceed all minimum standards related to the PGCSS. Develop and implement a data monitoring system to track and measure PGCSS processes and success. Serve as a liaison between the VA and community agencies, and participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver. Participate in direct caregiver and Veteran communication and assessments to identify and assess caregiver stress and burden and develop interventions to include one-to-one support and group support. Complete training to become a REACH VA Coach and a Health and Wellness Coach, and provide coaching to individual caregivers and also in group settings. Develop the annual series of support groups, and also facilitates groups as appropriate. Provide education through presentations on caregiver issues to VAMC employees, caregivers, Veterans, and other stakeholders. Serves as a liaison between the VA and community agencies and participates in caregiver support activities in the local community and stat to promote and recognize the role of the caregiver. Plans and implements an annual caregiver Resource Fair and an annual Caregiver Summit, along with the November Caregiver Support Month events. Promotes the VA Caregiver Support Program in coordination with the local VA Public Affairs Officer. Collaborates with clinical staff and program managers and maintains knowledge of current federal and state legislation. Assists with processing applications as needed for the Program of Comprehensive Assistance for Family Caregivers. Develops a library of caregiver support information material for distribution through the various inpatient/outpatient and specialty clinic settings. Performs other related duties as assigned by the Caregiver Support Program Manager and the Chief of Social Work and Care Transitions. Work Schedule: Monday-Friday, 8:00am-4:30pm Permanent Change of Station (Relocation Assistance): Not authorized. Pay: Competitive salary and regular salary increases. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not available. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Preferred Experience: Two years of medical social work. LCSW licensure Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to work with multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
