    Empowering People's Independence (EPI), provides support for people with developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion.

    Job Purpose:

    The primary function of this role is to assist the supervisor with the continuous direction and day-to-day operation of the Com Hab program.

    Essential Functions:

    + Maintain the health and wellness of the individuals served, to include understanding and following all safeguards and plans to provide the appropriate level of support in various situations.

    + Complete pre-Life Plan staff action plan review form and attend and participate in Life Plan meetings.

    + Attend Intake Committee meetings, and complete home visits as part of the intake process.

    + Add new referral information to the electronic record system.

    + Ensure all billing components have been completed prior to the start of service.

    + Maintain the health and wellness of the people served, including understanding and following all safeguards and plans to provide the appropriate level of support in various situations.

    + Complete pre-Life Plan staff action plans, review form and participate in Life Plan meetings.

    + Attend Intake Committee meetings and complete home visits as part of the intake process.

    + Add new referral information to the electronic record system.

    + Monitor all required documentation in accordance with Office for People with Developmental Disabilities (OPWDD) regulations.

    + Provide assistance and instruction on a daily basis to support Staff Action Plans (SAP)

    + Provide support and assistance to Direct Support Professionals (DSP’s), to include completing spot checks via phone or in person.

    + Ensure all shifts have proper levels of coverage.

    + Ensure all required company reports are completed per specification.

    + Exercise sound judgment as a matter of course and hold protected health information, to include HIPAA and other sensitive information in the strictest confidence in accordance with HIPAA and other company polices.

    + Assist with staff meetings as requested.

    + Demonstrate competencies of role as defined by OPWDD as identified during departmental orientation.

    + Follow up on all employee documentation to include staff action plans, notes, monthly summaries, electronic visit verification forms, to ensure they are accurate and current.

    + Report all policy violations to appropriate personnel.

    + Attend all staff meetings.

    + Promote workplace safety.

    + Perform other duties as assigned by Com Hab Supervisor and/or designee within scope of practice.

    Non-Essential Functions:

    + Participate on various committees as able.

    + Attend all meetings as required.

    + Assist in On-call rotation as needed.

    Compliance:

    + Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.

    + Must be willing to complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.

    + Must adhere to all departmental /company policies & procedures at all times and ensure compliance with direct report.

    Education and Experience:

    + Minimum High School Degree or GED with one (1) year experience in human services. Associates or Bachelor degree preferred in a related field with one (1) year experience in human services preferred.

    Computer:

    + Strong Working Knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams, and electronic medical record systems

    Qualifications

    + Travel is required for this position. A valid NYS Driver’s License is required to operate agency vehicle and/or personal vehicle.

    + Mature interpersonal/professional skills to effectively collaborate, resolve issues, and maintain confidentiality.

    + Solid verbal and written communication skills and ability to communicate with all persons supported and employees within various levels of the organization.

    + Demonstrated ability to use poise, tact, and diplomacy, during difficult and/or stressful situations.

    + Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.

    + Ability to train in and perform all required trainings for this position, including Strategies for Crisis Intervention and prevention-revised (SCIP-R), CPR, First Aid if applicable per department’s requirements.

    Supervision:

    + Oversight of approximately forty (40) Direct Staff Professionals (DSPs), to include assisting with recruitment, hiring, departmental orientation, training, work schedules, and managing performance.

    + Maintain required staffing levels of supervision of participants.

    + Ensure employees are compliant with all required training via the learning management system and in-person.

    + Must be able to talk, hear, stand, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.

    + Specific vision abilities required by this job include close vision and ability to adjust focus.

    + May be exposed to strong odors from time to time.

    + Must be able to lift and carry up to (50) lbs.

     

    While performing the duties of this position, exposure to changing weather conditions will occur.

     

    Pay Range : $22.00/hr - $27.50/hr
